Worker Dies in Blender at Meat Plant

CLACKAMAS, Oregon —  A cleaning employee was killed when he fell into a running blender at a meat-processing plant, according to NBC News.

The worker was identified as Hugo Avalos Chanon, 41, of Portland.

Authorities were called just before midnight on Saturday to the Interstate Meat Distributors for a report of a death. Several workers were cleaning the equipment when Chanon fell into a meat blender that was running.

Interstate Meat Distributors was cited for several safety violations discovered between October 2012 and January 2013, according to the Occupational Safety and Health Administration.

The cleaning company Chanon worked for, DCS Sanitation Management, was inspected in 2001, 2002 and 2004. OSHA found no violations.
