EASTLAKE, Ohio --  Police say Mindy Sotak, 29, of Eastlake, was arrested over the weekend for OVI with her kids in the car.

Sotak, a mother of two, is facing child endangerment charges for the second time in less than a month.

"She was witnessed pulling out of a parking spot at Walmart and hit another vehicle. After she hit the vehicle, she proceeded to exit the lot," said Chief Larry Reik, with the Eastlake Police Department.

According to witnesses, Sotak's three and five year old children were in the backseat.

Dispatch: "911. What is your emergency?"

Caller: "I need an officer at the Walmart in Eastlake."

Dispatch: "What's the matter?"

Caller: "There is this lady. I swear to goodness, she has two young babies in the car. She is drunk. She just literally backed into... right into a minivan."

According to the responding officer, Sotak appeared to be drunk.

"He noticed her putting gum in her mouth. He opened the car and while speaking to her, as she spoke she showed different signs of intoxication, including glassy eyes, a thick tongue," added Chief Reik.

Once she was booked, Sotak refused to cooperate and had to be physically restrained by officers.

Sotak is no stranger to the Eastlake Police Department.

She was arrested and charged with child endangering after another incident involving her children a couple weeks ago.

Police say back on May 1st, police found her passed out inside her home while the family dog attacked her three-year-old son in the backyard.

"In both cases we released the kids to another family member," said Chief Reik.

Sotak now faces two more counts of child endangering, in addition OVI, failure to comply, and hit-skip.
